| About us | We have served a longtime mission of outreach, education and assistance to help our community plan ahead for decisions about end-of-life care. Our certified staff provides facilitations, workshops, consultations and training courses that guide and empower individuals, families and healthcare providers to communicate, document and honor wishes. Empath Choices for Care helps families and the healthcare professionals who serve them with the advance care planning process. Our purpose is to educate and empower people to communicate and document their health care choices so that their wishes are honored. We provide educational programs, resources, and training to healthcare and legal professionals, clergy, community organizations and the general public on healthcare decision-making. Originally launched as an arm of the Florida Chapter of the American College of Cardiology, and originally known as Project GRACE, Empath Choices for Care was established through a partnership of physicians, care providers, ethicists and concerned citizens dedicated to changing the approach to end-of-life healthcare to a holistic, compassionate one that respects human dignity and the individual’s best interests and personal wishes. Founders Dr. Lofty Basta and Dr. Henry D. McIntosh (both cardiologists) were passionately dedicated to empowering individuals and their loved ones to properly document their end-of-life medical decisions based on their own ethical, cultural and spiritual beliefs and values, affording them peace of mind, dignity and respect. |
